New Patient Information

We welcome new patients to our practice and are honored that you’ve entrusted us with your neurosurgical or spine care.
  • What to Bring to Your First Appointment:
  • To ensure a smooth check-in process, please bring the following:
  • Completed New Patient Paperwork (see below to download)
  • Insurance card(s)
  • Valid photo ID
  • Referral form, if required by your insurance
  • Relevant medical records, imaging studies, and test results
  • List of current medications including dosage and frequency
  • Any questions or concerns you’d like to discuss with the provider

Please arrive 30 minutes early for your appointment to allow time for check-in. Please contact our billing specialist (573) 339-7472 for more information.

Insurance and Billing Information

Regional Brain & Spine accepts MOST major insurance plans. Please verify with your insurance prior to your visit to make sure our providers are in network with your specific plan.

  • We Accept:
  • Commercial Insurance PPO
  • Medicare and Medicare Advantage Plans
  • Workers Compensation claims with authorization.
