Office hours
Appointments
We make every effort to see patients at their scheduled time, so please arrive promptly for your appointment. If you arrive late, you may be rescheduled as a courtesy to our other patients. Please understand that emergencies or unforeseen circumstances may cause us to be behind schedule at times. If you are unable to keep your reserved appointment time, we ask that you provide us with at least 48 hours’ advance notice. Should a true emergency arise, please contact our office as soon as possible to reschedule your appointment.
What to bring
- Insurance card(s)
- Photo ID (Driver’s license)
- Diagnostic imaging on film or disc (if you were given these files)
- Complete list of medications you take
- Completed new patient registration forms (these will be mailed to you or you can download them here)
What to bring to returning appointments
- Diagnostic imaging on film or disc (if you were given these files)
- New medications you have begun taking since your last appointment
